Liability Waiver
 
I am aware that participation in the DBHOOPS LLC Summer Basketball Camp has some inherent risks and injury can occur. The Center will make every effort to contact me in case of an emergency. I authorize the staff of DBHOOPS LLC to act in my best interest and obtain necessary medical care for my child in the event of an emergency.  On rare occasions these injuries can be serious. In consideration of my child being allowed to participate in the DBHOOPS LLC Summer Basketball Camp, I, the parent/guardian, assume the risk of all injury and agree not to sue DBHOOPS LLC Academy, Monsignor Edward Pace High School, Archbishop Thomas Wenski, The Archdiocese of Miami, the camp directors, coaches, assistant coaches, agents, or volunteers for any and all injuries caused by or resulting from participating in the DBHOOPS LLC Summer Basketball Camp. By signing this waiver, I also authorize the use of pictures of the above-named participant to be posted on the DBHOOPS LLC web site or advertising social media.
